Вакцинация против вирусного гепатита B пациентов, перенесших трансплантацию печени по причинам, не связанным с гепатитом B

Цель работы – сравнение эффективности вакцинации против вирусного гепатита В (HBV) в группах больных, перенесших ортотопическую трансплатацию печени (ОТП) по причинам, этиологически не связанным с HBV, с использованием гранулоцитарно-макрофагального колониестимулирующего фактора (ГМКСФ) в качестве адъюванта и без адъювантов.

Материалы и методы. Вакцинация против HBV проводилась 18 больным, перенесшим ОТП в связи с заболеваниями печени, этиологически не связанными с инфекцией HBV. Девять пациентов получали двойные дозы вакцины по схеме 0, 1 и 2 мес с адъювантом (молграмостим). Группу сравнения составили 9 больных, получавших двойные дозы вакцины без адъюванта.

Результаты. В группе больных, вакцинированных по схеме с адъювантом, у 4 пациентов (44,4%) наблюдался значительный иммунный ответ (анти-HBs > 200 мМЕ/мл). Ни у одного пациента из группы сравнения такого ответа не отмечено (p=0,037).

Выводы. Применение молграмостима в качестве адъюванта при вакцинации против HBV больных, перенесших ОТП, увеличивает ее эффективность.
